Please complete Part 2 of the FTC Facility Request Form once you have completed Part 1 and FTC Staff have confirmed your rental date.  Complete Part 2  three (3) weeks or more before your confirmed rental date.  You may complete Parts 1 and 2 concurrently if you already know the details of your rental.
